Challenge: What kinds of digital and human interventions can empower patients and caregivers (heads of households and non-family caregivers) during critical healthcare transitions?

Process: We engaged in extensive user research to gain a better understanding of those (usually heads of households) caring for others at various life stages and the problems they faced by throughout patients' healthcare journeys.

Solutions: The team produced Digital Patient Experience, an app providing a humanized, user-centered digital experience that addresses common problems faced by caregivers. The app offers easy, on-demand access to medical history and supportive online communities. We also created Caregiver Experience, a system of support services that provides information, coaching, and resources to help caregivers navigate complex health transitions over the long term.
