Replacing Lost or Stolen Documents

If any of your important documents are lost or stolen, you should make an appointment to see International Student Services immediately. In some cases, a form I–102 (Request of Lost Document Form) will need to be submitted, and there is a fee. It may take several months to receive the replacement document.

If you are leaving the USA and you have lost your I–20 or DS–2019 a new one can be issued to you after you submit new financial documents to International Student Services.
