Replacing Lost or Stolen Documents
If any of your important documents are lost or stolen, you should make an appointment to see International Student Services immediately. If you lose your I-94, form I–102 (Application for Replacement/Initial Nonimmigrant Arrival-Departure Document) may need to be submitted, and there is a fee. It may take several months to receive the replacement document.
If you are leaving the USA and you have lost your I–20 or DS–2019 a new one can be issued to you after you submit new financial documents to International Student Services.

A new I–94 may be issued to you automatically upon your reentry into the USA. Please consult with an International Student Advisor.
If you have lost your passport, you must contact the consulate representing the country that issued your passport.
