Recommendation Form

Online recommendation form
Applicants are strongly encouraged to use the online application/recommendation system.  After you enter the names of three recommenders in the fields provided, the system will e-mail an invitation to them to submit their letters online. The process is easy and offers recommenders two ways to submit their letter: 1) writing directly on the secure web page or 2) by uploading a document file to the online form.

The alternative: a paper recommendation form.
If your recommender prefers to submit his or her letter via regular mail please download the Recommendation form (Adobe PDF). Carefully read the directions, completing the applicant’s portion, and then forward it to the recommender with a stamped envelope bearing the following address:

NOTE: When mailing the recommendation letter to the Admissions Office the recommender must sign the across the back flap of the envelope after it is sealed. The admissions office will not accept envelopes without a signature across the seal.
